With my 3 kids we have done the Sacagawea Dollar with all of them so far, Has worked well so far, Coins are uncommon enough that the kids think they are neat. You could try and get some of the Presidential Dollar Coins that they started making last year.
As far as how much per tooth, some kids in the area are getting $5-20 per tooth....At that rate I would be charging them the copay on dental..
